Open Hand Atlanta
Senior clients should contact the Office of Aging for client criteria and intake procedures. Case managers with Visiting Nurse Health System will complete the client assessment, then forward a communication form to Open Hand confirming enrollment and notifying us of delivery details and meal type information for each client Contact the Fulton County Office of Aging at 404/730-0184.
